Insurance Based Payments:

My company is an approved Provider of Rehabilitation Services for the Ohio Bureau of Workers Compensation. Please make sure all forms are completed before visit. Client will be given a 45-50 min rehabilitation session based on Ohio Bureau of Workers Compensation rules and regulations of billing for services.

Direct Out of Pocket Payments:

The average cost per session is $40 for 45mins. HSA Cards Accepted. Individual invoices can be printed upon request.

Please give 24 hour notice to cancel your appointment. Rehabilitation staff reserves the right to cancel or reschedule your appoint time due to unforeseen circumstances beyond control of Colored Waters.  This may include pool safety controls, operation of the pools and pool closings due to weather or hygiene issues.  Pool availability subject to change within a 24 hour time period of client's scheduled appointment time.
