RESERVATION POLICIES:

Please respond to the calendar “invitation” when it is email to you as soon as possible as to prevent overbooking or loosing your time slot.

CANCELLATION of APPOINTMENTS POLICY:

Please call therapist within 24 hours of appoint time if you need to cancel. If accident occurs on the day of your appoint, please contact your therapist by phone (text or email if person does not answer phone immediately) as soon as possible. Please leave voicemail message of your absence.

Colored Waters reserves the right to cancel or reschedule appoints due to inclement weather, family/ personal emergencies or upcoming holiday breaks.

POOL RULES AND CLOSURES:

Every facility has different pool times and closures subject to moments notice. Colored Waters is not responsible for pool time changes and pool closures that is at the discretion of the facility and management of that facility. Colored Waters is not responsible for lifeguards being present at pool facility. However, many times pools will close or change times at moments notice due to the availability of Lifeguards on shift. Please be patient as Color Waters finds new location for that day’s appoint session or rescheduling of appointment session. Facilities will close due to inclement weather. Please watch for facility closures on news feeds or Color Waters will attempt to contact you by way of phone or email for facility closures due to weather.

POOL FACILITIES:

Colored Waters is not held responsible for the cleanliness of the facility. If the facility is not to your satisfaction please inform the facility staff at the front desk. Colored Waters is not responsible for devices used to enter into the pool such as stairs and lift chairs. This is the responsibility of the facility that these such devices be at working order. Please make sure you bring a locking device (combination lock) or leave valuables at home. Colored Waters is held responsible for lost or stolen items. Please bring plastic bags, shower shoes, personal hygiene products and towels as many of the facilities we use do not supply these items.

MEDICAL INFORMATION, INSURANCE INFORMATION AND DOCTORS ORDERS:

In accordance to HIPPA and FERPA Laws, all medical records are stored at home office of Colored Waters. patient information is not shared with other companies or facilities for the purpose of advertising and solicitation. Doctor’s orders can be faxed or hand delivered at time of first initial consultation by the Occupational Therapist.

PAYMENTS AND CLUB MEMBERSHIPS:

Payments are due at time of service. Record of payments will be kept by facility and your therapist. Patient must sign attendance sheet at the time of each session. It is preferred that you have and must be able to financially maintain a facility membership. However, it is not a requirement. Colored Waters as established a partnership with many facilities to offer to our patients low cost or reduced membership rates. These rates will be given to you at the time of your second session. Your first session, you will be a guest of Colored Waters, LLC.
